Community First encourage and support collaborative activities, helping to secure the best health and wellbeing outcomes for people and rural communities.

We aim to connect community groups, charities, social enterprises and not for profit organisations with each other.  We also focus on strengthening links with statutory partners including NHS Trusts, Clinical Commissioning Groups (CCGs), Local Authorities.

Health and wellbeing is undergoing rapid, continuous change highlighting the need for regular, credible communication, engagement and representation.

We encourage regular exchange of information, ideas and views on health and wellbeing to assist charities and community groups to deliver timely, vital services.

The Wellbeing Team - 01905 766124 

The Wellbeing Assistants,  employed by Community First, support people with their mental wellbeing needs as part of the wider developments of the Here 2 Help service . Despite the current Worcestershire Wellbeing Hub contract ending, the phone number 01905 766124.

Anyone over the age of 16 can self refer via the phone number.

The Wellbeing Team offer the following:

  • direct receipt and processing of email and telephone requests from integrated services, including other VCS organisations, GPs, Local Authority, NHS, Neighbourhood teams, Social Prescribers and other professionals, as well as continued direct self-referrals.
  • have conversations  with callers to ensure they are offered appropriate support
  • direct callers to appropriate wellbeing programmes, including the learning opportunities of the Herefordshire and Worcestershire Wellbeing and Recovery College
  • signpost callers to wider community and voluntary sector support as appropriate
  • ensure callers are aware of and able to access self-help materials as appropriate
  • where clinical support is required, direct callers to Worcestershire Health and Care NHS Trust or the Neighbourhood Mental Health teams or crisis service as appropriate
