Sue Harris and the Mental Health Consultative Group would like to repeat the questionnaire originally sent out last December which informed the Health and Social Care Partnership Board of the main issues affecting them. Six months on they would like to repeat the questionnaire which will show if mental health service users’ issues/concerns have changed. It will also again provide information which could be fed into the new Health and Wellbeing Board.
Below is a copy of a questionnaire that they would like you to distribute to your services users and have highlighted the areas where you will need to add your own organisation’s details and your contact to send these back to. They have asked if they could be completed by the 15th of July and if you could send the responses back to Polly Lowe by the end of the 20th of July I would be very grateful.
