Managing New Realities brings together the worlds of health, social care and housing. Now in its fifth year, this unique event enables these sectors to meet and work together to forge a common agenda to produce better and more integrated care and support services.This year we focus on commissioning and delivering personalised care - a principle now espoused across the political spectrum - and ask the crucial questions: how is it possible to reconcile individual empowerment and community-wide outcomes; can services tailored to the individual really be as cost-effective as those commissioned in bulk; and how will the workforce have to adapt?
