Community First & Worcestershire Healthy Minds win Partnership and Integration Award

At the Worcestershire Health and Care NHS Trust staff awards, held on 20th October, the Partnership and Integration Award was won by Community First and the Worcestershire Healthy Minds team for creating the Wellbeing Hub as a link to community wellbeing support and new self-referral pathways into the primary care mental health services.

The latest phase of the development of services offered through this service is the Peer Supporter Service which has been set up to help ease the changeover from secondary to primary care support  for those people who are making that transition.   Moodmaster groups are now well established across the county with many participants reporting that it has helped to build their confidence as well as giving them helpful tools and suggestions to improve their mood and daily lives.
